It can be difficult and overwhelming determining what services are available and how to access them. Care Coordination services relieve some of that burden by guiding you through the maze of health care, financial and social services available in our Alaska communities
A variety of programs and services are available in the community to assist elders of all income levels and varying degrees of frailty. Using a care coordinator ensures that you or your loved one knows all the options available and are utilizing the right combination of services to receive the best care possible.
Care Coordination services are for those who require more than one service at a time or for those who need help accessing the Alaska Medicaid Waiver Program.

For more than 8 years, Liz has been working in Care Coordination and focusing her efforts on helping people get onto the Home and Community Based Waiver and working with the in-home folks who do not qualify for the waiver. She has spent a lot of time serving our rural communities.
Liz works with seniors, and with people who have Intellectual and Developmental Disabilities. She is often inspired by the people she works with for their wisdom, their humor and their ability to overcome obstacles.
