Mini Grants
Eligibility Instructions Application Due Date
Money for services and items is available for people with Alzheimer’s disease or related dementia (ADRD).
The grants must provide individuals diagnosed with ADRD essential items which will directly improve their quality of life and increase their independent functioning.
Uses can include:
- medical
- dental
- vision
- hearing services
- supplies
- therapeutic devices
- adaptive equipment
- caregiver respite
- accessibility improvements or supplies
- services not available from any other funding source.
Who is eligible?
A person may apply for a mini grant as:
- a beneficiary (person affected by the disorder)
- or as an applicant (a family member, care coordinator, or guardian filling out the form on behalf of the beneficiary).

The mini grant program is funded by a grant from the Alaska Mental Health Trust Authority and grants are distributed by Alzheimer’s Resource of Alaska.
How to apply
- Submit a completed mini-grant application.
(You may need Acrobat Reader to view & print this application.) - Attach a written estimate from the vendor for the cost of the item(s) or service(s) to request for the beneficiary.
Application Due Date
Mini grants are awarded monthly. Applications must be received by the close of business on the first Tuesday of each month. If the deadline falls on a holiday, the deadline rolls to the next business day.
This months application is due by 5 pm, 01/07/2025
